COPYRIGHT APPLICATION QUESTIONNAIRE
Please answer the following questions to the best of your
ability and email, fax or mail the completed sheet to us.

Full
name and address of the person or entity claiming ownership of the work.
The person who created the work may not be the same person who owns
the work.

Full
name, birthdate and citizenship or domicile of all authors.
·
Did
an employee create the work?
·
Was
an independent contractor to create or help create the work? If so, was the
role one of creation or of setting down someone else’s creation? Was there a
written agreement used? Was the copyright ownership issue addressed? If a
contractor was used and there has not been an assignment, we need to draft a
copyright assignment agreement and send it to you. Our fee for this is $150. A
written transfer is the only way to transfer the copyright to the owner.

owner is not the author, how did owner obtain ownership of the copyright?

Title
of the work, any previous or alternative titles of the work.

Describe
briefly the work and the nature of the author’s contribution to the work (did
the author create all/part/revise/translate the work?).

Is
the work a revision of an earlier work?

Year
in which creation of the work was completed?
·
Date
(month and year) and country in which the work was published, if at all?
Publication means distribution of copies of the work to the public, even if it
was only to several people.

If
work is a compilation or derivative work, identify preexisting work on which it
is based or incorporates, and, describe the additional material the copyright
claim covers.
· Has this work been previously registered with the Copyright Office?
